Het herstellen van verloren tandweefsel vraagt niet alleen om een esthetisch en functioneel eindresultaat, maar ook om een voorspelbare en efficiënte werkwijze. Digitale en analoge technieken maken het tegenwoordig mogelijk om een zorgvuldig ontworpen opwas nauwkeurig te vertalen naar de mondsituatie. De stempeltechniek met transparante siliconenmallen en verwarmd composiet biedt daarbij interessante mogelijkheden. Door gebruik te maken van een vooraf vervaardigde wax-up kan de gewenste anatomie gecontroleerd worden overgebracht, terwijl de behandelaar tegelijkertijd profiteert van een gestandaardiseerd en reproduceerbaar proces.  In deze casus wordt de behandeling van een patiënt met ernstige frontale en posterieure slijtage beschreven. In 2013 won hij de 3M Espertise Talent Awards met het ‘Digitaal Rehabilitatie Concept’ en in 2015 ontving zijn artikel ‘Tooth Wear: A Systematic Review of Treatment Options’ de Glen P. McGivney Scientific Writing Award voor beste systematische review in 2014. Erik-Jan is bestuurslid van de Dutch Academy of Esthetic Dentistry (DAED), mede-oprichter van KARMA. Dentistry en zit in de Raad van Advies voor InterCongress. | | --- | --- | | **Voorspelbaar en doeltreffend** De 52-jarige Hendrik werd naar mij verwezen voor de behandeling van gebitsslijtage. Slijtage was met namelijk aanwezig in het front (figuur 1-5) en attritie leek het grootste aandeel te hebben in de oorzaak ervan. Daarna werd er door het tandtechnisch laboratorium (Oral Design Center Blaricum) een opwas (wax-up) gemaakt. Deze opwas werd omgezet in de mond met een tijdelijk vulmateriaal ([Luxatemp Star, DMG](https://www.henryschein.nl/nl-nl/Search.aspx?hsec=1&searchkeyWord=luxatemp+star)), dit wordt ook wel de mock-up genoemd. Tijdens het verwijderen van de mock-up kan men goed zien om hoeveel materiaal het eigenlijk gaat (figuur 8). ![8-ST](https://info.henryschein.nl/hs-fs/hubfs/8-ST.jpg?width=1298&height=866&name=8-ST.jpg) *Figuur 8* De volgende zaken zijn van belang om met de stempeltechniek de opwas goed om te kunnen zetten in de mond; (1) een mooie en duidelijke opwas die stopt boven het tandvlees en die interdentaal de elementen niet laat versmelten, (2) stevige en gedetailleerde mallen van transparant siliconen in een gelijkmatige dikte, (3) bij een relatief grotere opbouw met weinig stops een alternerend model en een alternerende mal, (4) verwarmde mal en verwarmd composiet en (5) het uitblokken van interdentale ruimtes en buurelementen. Om een gelijkmatige dikte van de transparante siliconen ([Clearform X, Shofu)](https://www.henryschein.nl/nl-nl/dentaal/p/afdrukmaterialen/additiesiliconen/clearform-x-2x-50-ml-en-8-mengtips/9189241) te kunnen krijgen, werden eerst putty-stops in een partiele metalen lepel (zonder rimlock) gemaakt, daarna werd de transparante siliconen aangebracht op het model en in de lepel (figuur 9-10). Op dit model wordt ook een siliconen mal gemaakt en deze mal wordt de eerste keer gebruikt. Daarna werden de oude vullingen verwijderd en alles gezandstraald met 29um aluminium oxide (AquaCare Twin, Velopex of kies [Prepstart van Danville](https://www.henryschein.nl/nl-nl/dentaal/p/klein-equipment/diverse-klein-equipment/prepstart-200-259-00/212854)). Op het moment dat de 46 en de 44 goed zichtbaar werden door de mal heen, was het aannemelijk dat de mal goed op zijn plek was gekomen en kon worden gestart met de polymerisatie. Wederom werd de mal gevuld en aangebracht (figuur 18). Het verwijderen van alle overmaat is altijd veel werk, ook in deze casus.
